As an Academy, we take photos of classes and students to help document the wonderful memories each semester and better advertise our program. Please sign the below form to consent to photos/videos being taken of your child in their class.

Though we emphasize safety in each of our classrooms and regularly inspect all equipment, we want parents and students to acknowledge and accept the liabilities that can come with taking classes in the arts.
